Decision Support Model for Selecting Health Tourism Destinations in Indonesia Using IDOCRIW–MARCOS

Abstract

Indonesia has considerable potential to develop into a leading health tourism hub in Southeast Asia, supported by its expanding network of accredited hospitals, growing pool of medical specialists, and rich tourism assets. However, prospective medical travelers and policymakers still lack a systematic, data-driven instrument for comparing candidate destinations across the many, often conflicting, factors that shape such a decision. This study proposes a hybrid multi-criteria decision-making (MCDM) model that integrates the Integrated Determination of Objective Criteria Weights (IDOCRIW) method with the Measurement of Alternatives and Ranking according to Compromise Solution (MARCOS) method to rank Indonesian cities as health tourism destinations. Ten candidate destinations (Jakarta, Bali, Bandung, Surabaya, Yogyakarta, Medan, Makassar, Semarang, Solo, and Manado) were evaluated against ten criteria covering healthcare quality, cost, accessibility, accommodation, and tourism attractiveness. IDOCRIW, which combines the Criteria Impact LOSS (CILOS) technique with Shannon entropy, was used to derive objective criteria weights, giving the highest importance to accommodation availability (0.171), medical service cost (0.152), and transportation accessibility (0.125). The MARCOS method was then applied to rank the alternatives relative to ideal and anti-ideal solutions. The results show that Jakarta (f(Ki) = 0.699) ranks first, followed by Bali (0.692) and Surabaya (0.683), while Manado (0.610) ranks last. The proposed model offers a replicable decision-support tool for the Ministry of Tourism, hospital associations, and destination-marketing organizations seeking to prioritize investment and promotion strategies for Indonesia's health tourism sector.
